Proof of Coverage Inquiry

POC Timeliness Reporting Requirements |
Transaction Received Date | NCCI’s received date is used to determine timeliness on POC transactions accepted by the state. |
Policies | Must be filed within 30 days after the Policy Effective Date. POC Notices may be submitted if the policy will not be issued within 30 days after the Policy Effective Date. This does not eliminate the need to submit the policy. |
Cancellations | Cancellation Due to Nonpayment of Premium—Must be filed at least 10 days before the Cancellation Effective Date. Cancellation Due to reason other than nonpayment of premium—Must be filed at least 30 days before the Cancellation Effective Date. |
Reinstatements | Due within 30 days after the Reinstatement Effective Date. |
Nonrenewals | Oklahoma is not a continuous coverage state. Nonrenewals are not required for POC – the Policy Expiration Date is recognized by the Oklahoma Workers Compensation Commission. Per the state’s request Nonrenewal transactions are not sent through POC. |
POC Data Reporting Requirements |
Data Elements: -
Industry Code—A valid 2017 or 2022 North American Industry Classification System (NAICS) code must be reported for policies effective 1/1/2017 and later. For policies effective between 1/1/2012 and 12/31/2016, a valid 2012 NAICS code must be reported.

Number of Employees—Report, if available, on Address Type Codes 1, 2, and 6
- Refer to the POC Data Element Requirements Chart located in NCCI’s
Policy and Proof of Coverage (POC) Reporting Guidebook for all requirements
Address Reporting—No Physical Location: - When there is no physical location in the state, Oklahoma will accept Employer Names linked to:
- Address Type 6—No Physical Location
- Address Type 2—Location of Operations with an Out-of-State Address reported with State Code Link 35

POC Penalties & Appeals |
POC Fines | Per O.A.C. 810:25-3-1, failure to timely and accurately file a policy with NCCI is subject to a fine of not more than $1,000 per policy as determined by the Commission. |
Appeals | Decisions of Commission administrative law judges are appealable to the three Commissioners sitting en banc. Decisions of the Commission are appealable to the Oklahoma Supreme Court. |
